Care Coordinator

The Role:

Being a Care Coordinator with Helping Hands is about more than just rota management. You will be responsible for supporting the Manager with Recruitment, Business Growth and ensuring we are always delivering the highest possible standard of care.

Main Responsibilities:

  • Allocating the right carer to the right customer at the right time
  • Supporting the Care Manager with business development
  • Recruiting of the next generation of Helping Hands Care Assistants
  • Ensuring all carer and customer records are up to date
  • Liaising with families and other healthcare professionals to ensure the quality of care we provide is the best in the marketplace.

To ensure that all customers and carers receive a good quality service within the specified timescale. To communicate perm calls and variations to carers. Ensure excellent working relationships with healthcare professionals, customers and the next of kin.

 

 

Primary Responsibilities

 

·         To allocate perm calls / runs to carers ensuring continuity

 

·         To ensure that immediate calls are covered.

 

·         Communicate with customers when there are changes to calls.

 

·         Communicate with carers when there are changes to their work at short notice.

 

·         To plan and organize future cover up to generation and to ensure all carers allocated work is given to them in a timely manner

 

·         Complete Care Calls if no other carer can be provided

 

·         To ensure that the perms are at 3% or under within your postcode areas.

 

·         To take accountability for all conversions for new packages on CH

 

·         Ensure purchase orders are obtained for service and are kept up to date and recorded accurately.

 

·         Take responsibility for decision making in regards to new packages to enable the business to grow.

 

·         To plan and organise interview, shadowing & coaching for new recruits.

 

·         To conduct interviews and orientation for new recruits.

 

·         To conduct Supervisions and Direct Observations every 3  months or as required

 

·         To support the Branch Manager with customer visits and reviews as and when required.

 

·         Undertake New Business assessments and produce individual care and support plans

            and risk assessments for new customers if required.

 

·         Liaise with Recruitment  to ensure they are aware of staffing requirements

 

·         All carer and customers user records to be kept live and accurate on Cold Harbour.   

 

·         To support other postcode areas with Carers during holidays / absence

 

·         Cover Out of Hours Service on a Rota Basis

 

·         Support Branch Manager with Carer Recruitment

 

·         Be confident and capable to run the branch in the managers absence

 

·         Any other duties required by the needs of the business


 

Personal Specifications

 

They must have direct experience of planning care visits for customers. They will need to be confident with IT solutions and have an excellent track record in punctuality and attendance. Be flexible for the needs of the business and work effectively with team members and on their own. They must be self motivated and be passionate about the care sector, customer and carers. They must have a positive mental attitude and desire to learn more about the care sector. They must be confident in delivering all aspects of care including assessments.

 

Competencies

 

The successful candidate will be Customer focused and have an excellent telephone manner with good communication, interpersonal and influencing skills. Must be competent with Basic IT such as Word, Excel and Outlook. They need to be able to multi task and work well under tight deadlines and pressure.
